Oranga Tamariki doesn't know whether it has been implementing basic health checks for the children under its care, according to a new report from the Independent Children's Monitor.
The review reveals a lack of compliance and poor record-keeping for doctor and dentist visits.
The chief executive of the Independent Children's Monitor, Arran Jones, says the message is simple: caregivers must note down whether the child has been to the doctor - and if not, get them there.
